We’re thrilled to announce that the NHS Health Checks Pilot we have been a part of throughout the last 2 years are continuing again!

We’d like to say thank you and congratulations to One Wirral CIC for their support and for making this project happen, it's been great to offer health checks to the community and open up conversations around wellbeing, smoking and alcohol habits. We have been delighted to take them out on the road to deliver for other organisations members and staff in the last year and look forward to doing even more in the future!

What is an NHS Community Health Check with Make It Happen?

We deliver these health checks as a trusted partner of One Wirral CIC, but what do they entail?

There are a variety of questions about your lifestyle, which all feed into a measurement of your risks of developing Cardio Vascular related diseases and Diabetes. We cover these questions in a conversational, non-judgmental way, and the best thing about the health check software is that we can show visually how changes in habits or behaviours can have an impact on your health outcomes. There is also a blood pressure check, and a Cholesterol blood test, with the results available within minutes! Everything feeds into a visual report that belongs to you, and is shared with your GP. Any red flags, such as a very high blood pressure for example, mean that your GP will get in touch with you. They are a great way to not only check on your health but chat about your health too, with Health Checks generally taking 40/45 minutes.

We will be carrying them out with members of staff specially trained to deliver them for our community. Completely free, they are a risk assessment health check looking at diabetes and cardiovascular health, and are available as face-to-face appointments for anyone between the ages of 40 and 74 who doesn’t already have a formal diagnosis. Get in touch with Claire on 07458 302 051.
